Artist in Residency Permission Form + Application

invites artists touched by cancer to become a

JONS Artist in Residency

At JONS, we value the art of practicing medicine.

We also value Art as Medicine to lift the human spirit.

To honor these values, we welcome partnerships with artists who have been touched by cancer to tell their story and showcase their artwork.

Each quarter, we will appoint a JONS Artist in Residency. Artists will have their artwork featured on up to 3 covers of JONS as well as online in our JONS Art Gallery.

A personal statement, description of the artwork, or brief bio and link to their professional website will be shared in the journal and online on the “Meet the Artists” page.

Publication: Journal of Oncology Navigation & Survivorship (JONS)

I hereby acknowledge that I have given permission to Journal of Oncology Navigation & Survivorship (JONS), published by Amplity Health, to use my art and any accompanying text in print and in electronic media without time limitations. This permission also applies to any photos or images provided with my article.

This permission applies to the art and art description that was provided to and edited by JONS and was approved by me.

It is further understood that I am free to use this art in the future in the form submitted to JONS, but without any additional images or pictures provided by the journal and used in the final version and printed in JONS.

Furthermore, I may not authorize third-party requests for reprinting or republishing the art/article in the form as it appears in JONS, without permission from the publisher.
