The Dr. Harold Freeman One Team Award honors excellence in healthcare navigation by recognizing a team of professionals who exemplify compassionate, innovative, and patient-centered care. Named after Harold P. Freeman, MD, the founder of patient navigation and Editor Emeritus of the Journal of Oncology Navigation & Survivorship (JONS), this award celebrates teams who work collaboratively to guide patients through the complexities of care—from diagnosis to survivorship and beyond.

Awarded annually by JONS, the selected team will choose a charity to receive a $1000 donation. In addition, the team will be:

  • Recognized with a commemorative plaque
  • Featured in a JONS article and on JONS social media channels
  • Invited as special guests on signature Amplity podcasts (launching in January 2026)

This award is about more than outcomes—it’s about honoring the collective spirit of teams who break down barriers, build bridges to care, and uphold Dr. Freeman’s vision of equitable access and unwavering support for every patient.

Nomination Process

We invite colleagues, administrators, and fellow navigators to nominate a deserving navigation team actively involved in healthcare navigation in any field. The selected team will embody the highest standards of patient advocacy, interprofessional collaboration, and innovative practice in professional healthcare navigation.

Nomination Requirements

To be considered, a nominated team must:

  • Be composed of two or more members working collaboratively within healthcare navigation, or healthcare adjacent navigation (social, financial, advocacy, patient)
  • Currently work with patients in a care-based setting, in any field or specialty
  • Demonstrate a measurable and meaningful impact on patient outcomes, journeys, and experiences
  • Embody the principles of equity, access, compassion, and innovation for patients and patient experiences

Each nomination must include:

  1. Completed nomination form (below)
  2. 500-word description explaining why the team deserves the award, including specific examples of collaboration, innovation, removal of barriers to care, and patient impact. Eligible teams must have performed their nomination activities within the calendar year of their nomination.
    What to include: stories, how you learned of the nomination activity, relation to the care team.
    What not to include: any PHI, including names, images, or materials that could compromise patient anonymity.

Submission Deadline

All nominations must be submitted by:
Friday, October 31, 2025, at 11:59 PM ET

Late submissions will not be accepted. The winning team will be notified in advance and publicly recognized in the December issue of JONS.

Accepting the Award

Nominated teams will be contacted by a member of our staff and will be notified of their nomination and selection for the award. Marketing, communication and video / photographic image releases may be required to accept the award. Delivery of award materials and attributions will be discussed with the awarded teams after signing releases and acknowledgements.
