Coordination of Care/Care Transitions

Baylor Scott and White McClinton Cancer Center (BSWMCC) is a comprehensive community cancer center located in Central Texas offering medical and radiation oncology services, plastic and reconstructive surgery, and lymphedema therapy, with approximately 275 patient encounters daily.


Transitional care (TC) is a series of interventions aimed at improving outcomes for patients who are experiencing a change in care status, such as hospital to home.

Investing in a dedicated lay staff to facilitate admissions and discharges across care settings can lead to meaningful improvements in healthcare utilization and quality, and in the overall patient experience, according to results from a pilot program presented by Mark Liu, MHA, at the 2020 ASCO Quality Care Symposium.



A 21-year-old with carcinoma of the nasopharynx was first evaluated by the Head and Neck Oncology Service at Memorial Sloan Kettering Cancer Center (MSKCC) in April 2020.

The process of screening and/or detection to the actual diagnosis of lung cancer involves multiple specialties and diagnostic testing, which can be quite daunting for an individual and his/her family.
