Delivery of a personalized TS/SCP for cancer survivors is a widely endorsed standard of care.
The Northwell Health Cancer Institute consists of 12 disease site Centers of Excellence, one being the Breast Cancer Center of Excellence.
The impetus to create a survivorship program at our community hospital–based cancer center was our desire to attain NAPBC accreditation.
Cancer and its treatment can have long-lasting or late-onset effects. A robust survivorship program, founded on distress screening and survivorship care plans as recommended by the Institute of Medicine and Commission on Cancer (CoC), can offer support, guide treatment, and provide education in the years after completing cancer treatment.
Through a grant provided by the American Cancer Society, AltaMed Health Services (AltaMed) implemented a Breast Cancer Outreach and Screening Program with multiple elements. The overall program aims to provide breast health education and screening mammograms to eligible women who are patients at the West Covina, El Monte, and Santa Ana-Bristol clinics.

This work indicates that the use of technology can improve diagnosis of early-stage lung cancer in the ER setting within the general population.
In a recent discussion review on the number of women being screened at the Women’s Imaging Center, the nurse navigator noticed that there was no screening program for women in the inpatient setting.
Genetics and genomics are playing an important role in cancer risk assessment. The Novant Health Cancer Risk Clinic was designed to assist patients and healthcare providers in addressing the complex issues of cancer risk related to genetic mutations and/or familial history of cancer.
This project highlights the importance of nurse navigators and interdisciplinary collaboration and demonstrates how they positively affect healthcare and patient outcomes.
